To make an appointment, please call your physician's office directly.  Please have your insurance card or group number available.

If you do not have your physician's phone number, use your insurance plan's physician finder to locate his or her information. Office addresses and phone numbers are listed here.

If you do not have a physician and need a referral, we can help. Our helpline staff will ask you questions about your physician needs and preferences and can refer you to a physician you can trust.

If you already have a physician's order, you may contact us directly to make an appointment with any of our facilities listed here.
